Memo of Understanding (MOU)

Each California Local Education Agency (LEA) that administers the CHKS is required to sign a MOU that includes a Confidentiality Agreement. The MOU outlines the responsibilities of both the LEA and the RSC and the conditions that must be met in administering the survey. LEAs wishing to administer the CHKS must sign and return this form to the RSC via mail or fax, and follow all Sign-Up Instructions.

Note: Submission of the MOU alone does not sign you up for the survey. You must also make contact with your Regional Survey Center after they have received the MOU.

Request for Raw Data

This MOU outlines the responsibilities of both the LEA and the RSC for a request of the LEA's Raw Data Set. Raw data is provided in SPSS format to allow districts or counties to further their data analysis. Familiarity with SPSS and/or data analysis is required.
